Step Five: Bank Conformation Letter and Principle Individual Letter.

This step will vary depending on your company structure and what your CA has requested. In our ride along our CA GeoTrust has requested a Bank Conformation Letter because I did not file for a DUNS number and a Principle Individual Letter because evOwl.com is setup as a DBA. UPDATE 4/18/2016: Lesson learned, it is much faster and easier to get a DUNS number than to deal with a bank. I am still required to file the Principle Individual Letter but you can get your DUNS number in less than a day.

For our EV SSL ride along this is our last step that will require a trip outside the office. If all goes well this is our last step before we are sent our EV SSL. Remember to bring a copy of your DBA with you to the bank. Some banks will allow you to bank under your DBA name with a personal account so you do not need to setup a separate business account.

Bank Conformation Letter Instructions
  1. This letter must be completed, and signed by an official of the regulated financial institution where the organization maintains an active demand deposit account.
  2. If available, please submit on financial institution’s letterhead.
  3. GeoTrust will contact the financial institution to verify the information provided within this document.
  4. You can fill out everything in Yellow, allow the bank to fill out the Green parts.
  5. This letter is normally only requested if you did not file for a DUNS number.
  6. When finished, upload via the upload link you received via email.
  7. Demand deposit account is just another name for a checking account, a share draft account, or a current account.

Instructions to Customer
  1. Print out this form and complete items in yellow.
  2. This form can only be signed by the Organizational Contact listed on your Extended Validation (EV) certificate request, in the presence of a licensed lawyer, Certified Public Accountant, Notary Public or equivalent. The organizational contact must be an Owner/Principal Individual of the business.
  3. Bring three (3) forms of identification with you to the lawyer, accountant, or notary as follows:
    1. One widely recognized government-issued photo ID signed by the individual such as:
      – A Driver’s License
      – A Passport
      – A personal identification card
      – A concealed weapons permit
      – A military ID
    2. One secondary type of identification from a financial institution (photo NOT required) that includes your name, such as:
      – A valid major credit/debit card, provided that it contains an expiration date and it has not expired
      – A mortgage statement from a recognizable lender that is less than six months old
      – A bank statement from a regulated financial institution that is less than six months old
    3. One additional secondary type of identification (photo not required) that includes your name as follows:
      – A second identification document from a financial institution(may be the same type as one listed in B; e.g Two separate Visa cards can be used)
      – Recent original utility bills or certificates from a utility company confirming the arrangement to pay for the services at a fixed address (not a mobile/cellular telephone bill)
      – A copy of a statement for a payment of a lease provided the statement is dated within the past six months
      – A certified copy of a birth certificate
      – A local authority tax bill for the current year
      – A certified copy of a court order, such as a divorce certificate, annulment papers, or adoption papers.
  4. Instruct the lawyer, accountant, notary to read the instructions and complete the Acknowledgement.
  5. Sign your name in the presence of the lawyer, accountant, or notary.
  6. E-mail (as an attachment) the completed (original) notarized form to GeoTrust in order for Customer Support to verify the form prior to you mailing the original copies. The form and all 3 types of identification must be attached and attested by the notary, lawyer, or accountant.
  7. Only after Customer Support has verified the form is properly filled out, send the completed (original) notarized form and all 3 types of identification attested to by the lawyer, accountant, or notary by: First Class Postal Mail, Federal Express, or other equivalent means.
Do not mail the documents to your CA until they have reviewed the uploaded version.
Instructions to Notary or Lawyer
The document you are attesting to is part of the GeoTrust Extended Validation SSL certificate request process. Please note GeoTrust will contact you to confirm you performed the attestation.

Extended Validation SSL certificates are issued pursuant to strict guidelines published at www.cabforum.org (“EV Guidelines”). These EV Guidelines mandate that GeoTrust obtain verification of a principal individual associated with the business applying for the certificate. Specifically, section 14(a)(3) of the EV Guidelines.

  1. Modify this form where necessary to assure compliance with the laws of your jurisdiction. Use the backside of this form if necessary.
  2. Complete the acknowledgement.
  3. Request and examine at least three pieces of subscriber identification as follows:
    1. One widely recognized government-issued photo ID signed by the individual such as:
      – A Driver’s License
      – A Passport
      – A personal identification card
      – A concealed weapons permit
      – A military ID
    2. One secondary type of identification from a financial institution (photo NOT required) that includes your name, such as:

      – A bank statement from a regulated financial institution that is less than six months old

  4. You must attest on a copy of the of the current signed government- issued photo identification document that it is a full, true, and accurate reproduction of the original, and attach it to this statement.
